Studies investigated TEG®/ROTEM® for diagnosis of early coagulopathies (n = 40) or for associations with blood-product transfusion ( n =25)ormortality(n =24).Most(n = 52) were single-center studies. Techniques examined included rapid TEG® (n =12), ROTEM® (n = 18), TEG® (n = 23), or both TEG® and rapid TEG® (n = 2).

Four RCTs show benefit of blood product administration guided by thromboelastography (TEG), compared to traditional coagulation tests. These are all single-center studies, but they seem to give a coherent message: use of traditional coagulation tests leads to over-transfusion. The optimal TEG-based transfusion algorithm is unclear.

TEG has been used at our institution in surgical procedures to guide blood component transfusion in bleeding patients. We evaluated performance characteristics of TEG and ROTEM, compared suggested blood product for transfusions based on algorithms for TEG and ROTEM, and correlated those results with conventional laboratory tests.

For the purpose of this post we will focus on ROTEM as this is predominantly used in Canada.

TEG, ROTEM, and other VHAs are powerful tools that can guide your transfusion of blood products in real-time. Hopefully the simplified algorithms listed here will help you make use of this valuable information. There is growing evidence that application of TEG- or ROTEM-guided transfusion strategies may reduce the need for blood products, and improve morbidity in patients with bleeding.
